The LIFE program allows eligible seniors to live safely in their homes with access to the medical, social, and rehabilitation services they need. To be eligible, individuals must be at least 55 years old, live in the service area, need nursing home level care, and be able to live safely in the community with help from LIFE at the time of enrollment; a more detailed outline of the enrollment process can be found here: https://lifeviecare.org/eligibility-enrollment/

When individuals join the LIFE program, they will receive an individualized care plan that covers all the necessary services. Services include, but are not limited to: primary care, social services, transportation, recreational therapy, rehabilitation, and home health.

Individuals eligible for both Medicare and Medicaid will not have any costs for any drug, service, or care if approved by the LIFE health team. For individuals with Medicaid, they will not pay a monthly premium for the long term care portion of the LIFE benefit. For individuals with Medicare but who do not qualify for Medicaid, they will be responsible to pay for a monthly premium to cover the long-term care portion of the LIFE benefit and a premium for Medicare Part D drugs. For individuals not eligible for Medicare or Medicaid, they can pay privately.
