This organization provides opportunities and lifestyle choices for persons with intellectual disabilities, autism, and other persons with disabilities. This organization is located in Bradford and Tioga counties and serves individuals aged 18 and older who become active and valued participants in their communities. They provide a continuum of care, offering: community residential services, adult day services, an intermediate care facility, life sharing services, respite care, senior services, and health & safety services. Residents participate in health, recreational, exercise, and dietary programs. Residents also participate in the events and happenings of their communities. Services are funded through Medicare, Medicaid, or county programs. Eligibility and access to services are determined through an approved waiver process.
