Provides primary care to adults, children and families. It also offers specialized health services to adults with unique medical needs, including HIV and Hepatitis C. The health center also provides referrals to PHMC Health Dental and PHMC Health Pharmacy’s on-site services. Its team includes bilingual staff and offers interpretive services. Services include: primary and preventive care, behavioral health services, chronic disease management, podiatry care, women’s health services, substance use disorder treatment, dental services, pharmacy services, and social services. This location serves adults, children, and families regardless of their insurance status. A sliding fee scale is available for eligible patients based on family size and income.
