Public Health Management Corporation – Supportive Housing Services

Offers a comprehensive continuum of care designed to support individuals and families experiencing or at risk of homelessness. Services integrate health, social services and case management to address immediate needs and promote long-term stability. Through these comprehensive programs and services, PHMC strives to empower individuals and families, helping them achieve stability and self-sufficiency. Emergency Housing: ACTS 1 & 2 Emergency Housing Programs, and Love and Care Emergency Housing Program. Rapid Re-Housing: Continuum of Care (COC) Family Rapid Re-Housing Program, and First Foundation Rapid Re-Housing Program. Permanent Housing: PHMC-12, and HOPWA Tenant-Based Rental Assistance Program (TBRA). Services include: rental counseling and mediation, utility assistance and energy conservation workshops, child care systems navigation, credit counseling repair, referrals for legal services, assistance with VA and other public benefits, linkages to transportation assistance, parenting classes and children/youth programs, resume development and employment opportunities, and workshops on health/nutrition and senior housing/aging. Eligibility: adults aged 18+, families, and resident of a permanent housing facility that has contracted with PHMC to provide supportive case management services.
