Assisted Living Program: three-bedroom, ranch style homes, designed to support individuals with a history of mental health concerns. The individuals residing in this program typically have medical needs that require increased monitoring (such as dietary restrictions, additional support for daily hygiene, etc.) The program is staffed 24/7 and all referrals must be made to the York/Adams MH-IDD program.
Community Residential Rehabilitation Program: designed to assist adults with a mental health history live in a safe, home-like environment, while empowering each consumer to reach their highest level of independence. This program provides a structured “group home” setting, in addition to training and assistance with symptom management, interpersonal skills, and daily living skills.
Supportive Living Program / Community Hospital Integration Project Program: provides community-based housing and 24/7 staff assistance to individuals, formerly living in psychiatric state hospitals or other structured living settings. The program provides an array of support services such as medication monitoring, transportation, etc., to individuals living in a clustered apartment setting.
Supported Housing Program: individuals locate and secure their own apartment while Supported Housing provides the level of support services necessary for the individual to maintain their independence (assistance with transportation, daily living skills, etc.).
Social Rehabilitation Services: psychosocial rehabilitation program designed to provide an educational, recreational, and supportive environment for the enrichment and development of individuals with mental disorders. Their center offers daily living skills, vocational goals and socialization classes that help individuals to understand and accept their mental illness and live in their community of choice without recurring hospitalization.
Psychiatry Services (children and adult): medical services focused on diagnosing and treating mental health needs. They offer evaluation, diagnosis, and medication to help manage behavioral health symptoms. Though telehealth visits are an occasional option for some people, attending your appointment in an office on a regular basis is required so that they can ensure the best quality care.
Community Outreach Recovery Essentials: provides treatment and support for individuals recently discharged from a state hospital or to prevent hospitalization. Unlike traditional outpatient therapy, much of the treatment occurs in the individual’s residence or elsewhere in the community. They also provide mobile nursing services, monthly appointments with the team psychiatrist and on call consultative services.
Mental Health Outpatient Therapy Services: provides individual, family, and group therapy in a traditional clinic based model. Services are provided both in person and by telehealth. Their therapists are trained to work with individuals experiencing a wide variety of concerns such as anxiety, depression, trauma, relationship issues, among others.
Outpatient Substance Use Treatment: therapists focus on education, problem identification, increasing self-awareness, developing healthy coping skills and providing referrals where necessary.
Partial Hospitalization Program: delivers outpatient group services in a highly structured therapeutic environment for individuals who need more intensive treatment than is available through individual counseling or who are at risk for admission to a psychiatric inpatient facility. This service also helps individuals transition back into the community from a psychiatric hospital.
Peer Support Services: designed to help individuals living with or experiencing mental illness through the recovery process. By utilizing trained Certified Peers who are succeeding in their own recovery from mental illness, clients can learn and benefit from their real life experiences. This program allows clients to build a comfortable and open relationship with the Peer Specialists based on the idea that each has experience living with a psychiatric illness.
Student Assistance Program: this is for students who are having difficulty in school due to alcohol/drug use, depression, or other mental health concerns. SAP seeks to assist and refer these students to appropriate in-school and community resources. Once a referral has been received, TrueNorth Wellness Services works together with area school districts, students, and their families to develop a plan of action to offer support and resolve identified concerns.
