
Public Health Management Corporation – Medical Respite
Offers post-acute medical care for people experiencing homelessness who need medical care after a hospital stay. The benefits of medical respite programs include better health outcomes for patients, lower re-admission rates for several months and long-term cost savings for hospitals, insurance companies and communities. Services include a comfortable space for rest and recovery; medication management, follow-up appointments and linkage to primary care and other PHMC resources; case management to help connect patients with social services; and assistance with accessing post-respite housing. Eligibility: adults age 18+, experiencing homelessness, in need of post-acute recuperative care (includes prep for colonoscopy), independent in activities of daily living and instrumental activities of daily living (activities of daily living will be reviewed on a case-by-case basis). All patients will be screened for their ability to live safely in a communal setting.
